Payment Options

For Those With Healthcare Insurance

NSCHC accepts most health insurance plans. All co-payments are expected at the time of service. If you are unable to afford the amount owed, you may work with our Benefits Eligibility Counselor to determine if you are eligible for our sliding fee discount program.

For Those Without Healthcare Insurance

All patients are seen regardless of ability to pay. NSCHC will provide you with high quality healthcare services whether or not you have health insurance. A Sliding Fee Discount Program (SFDP) is available which provides discounts for our medical and dental services based on household size and income. The SFDP allows us to determine a fair discounted rate so that you pay only what you can afford.

You can download a copy of the application for the SFDP here, or request to have an application mailed to you. Please return the application before your first visit so that the correct discount can be determined.

GOOD FAITH ESTIMATE

A good faith estimate is available for any patient who does not have insurance, and schedules their appointment at least three days in advance.
